Origin and history

The Hôtel-Dieu Saint-Jacques de Toulouse came into being in the 12th century, with the foundation of two separate hospitals: the Novel Hospital (1225) and the Sainte-Marie de la Daurade Hospital (1130), the latter linked to the pilgrimage of Santiago de Compostela via the bridge of the Daurade. These establishments, managed by the Confrérie de Saint-Jacques from 1257 onwards, merged in 1313 under the name "Hôpital Saint-Jacques at the end of the bridge". In medieval times, their role is more religious than medical, welcoming pilgrims and sick in a context where technical knowledge is sorely lacking. The Saint James shells, still visible today, bear witness to this first vocation.

Between the 17th and 18th centuries, the Hôtel-Dieu experienced a major expansion, becoming the largest hospital in Toulouse. The expansions include the wing of the incurables (1702), a district dedicated to women in bed and the venereal sick (1750), as well as the wall of the wharf (1777) linking the Grave to the hospital. On the architectural level, the grand staircase of honor (1716) and an Italian glass roof are added, while the demolition of the bridge of the Daurade (1639) only leaves a pile attached to the building. These changes reflect the adaptation to the growing needs of a changing city.

In the 19th century, the hospital modernized its infrastructure: the main courtyard was symetricized (1864–66), and new buildings appeared, such as the Press Pavilion (1884) for sick children or the morgue (1890). The twentieth century marked a turning point with the gradual transfer of medical services to the hospitals of Purpan (1940) and Rangueil (1975). Ranked a historic monument in 1988 and registered as a UNESCO World Heritage Site in 1998 (paths to Compostela), the Hotel-Dieu now houses the administrative center of the CHU, medical museums, and a dermatological research center.

The history of the Hôtel-Dieu is also that of Toulouse, marked by the floods of the Garonne, epidemics, and the evolution of medicine. Its architecture, mixing medieval vestiges (Plate du Pont-Vieux), classical elegance (scaling of honour), and modern functionality, makes it a symbol of resilience and hospital innovation throughout the centuries.
